Re:      Deadline Extension for HealthcareTransformation Collaboratives Applications     

 

 

TheDepartment of Healthcare and Family Services is announcing that the deadlinefor FY2021 Health Transformation Collaboratives applications to be received hasbeen extended until April 12, 2021 at 5pm.

 

Asoutlined in PublicAct 101-650 and PublicAct 101-655 passed in January 2021, the program is designed to improve healthcareoutcomes, reduce healthcare disparities, and realign resources in distressedcommunities throughout Illinois. In particular, the program seeks to increaseaccess to community-based services, preventive care, obstetric care, chronicdisease management, and specialty care in these communities.

 

Thisprogram makes available as much as $150 million per fiscal year beginning inState Fiscal Year 2021. This funding is available to collaborations betweencare providers, including preventative care, primary care specialty care,hospital services, mental health and substance abuse services, andcommunity-based entities that address the social determinants of health.Priority will be given to collaborations that include safety net hospitals orcritical access hospitals, as well as culturally-competent, minority-owned or-controlled partners. Applications from all types of provider collaborationsand from communities across the state will be considered over the coming monthsand years.

 

Followingthe first round of awards, applications will again be available for a secondingfunding cycle in the fall. There will be opportunities to apply for funding insubsequent years as well. For the fall deadline and future funding cycles, HFSwill be offering consulting services to assist collaborations withapplications.

 

Theapplication and supportive materials are available on the Department’s HealthcareTransformation webpage. The Department encourages only thosecollaborations with well-developed plans to apply for the first round offunding. Applicants that have already made submissions are being informed thatthey may submit updated proposals before the new deadline if they choose.
