Re: 2018 Long Term Care (LTC) Cost Report Forms and Instructions
The purpose of this Informational Notice is to notify providers that the 2018 Long Term Care (LTC) cost report form and instructions are available on the Illinois Department of Healthcare and Family Services’ Cost Report webpage. The cost report is due on September 30, 2018 or 90 days after the close of the facility’s fiscal year, whichever is later.
The downloadable Excel cost report file is accessible at the link entitled Long Term Care – Long Form (Nsg Homes, Skilled Nsg Units and ICF/IID). Please use the 2018 cost report file and instructions. Printed copies of the report from the 2017 cost report or earlier files will NOT be accepted.
When the report is complete, please submit via email or CD to: HFS.HealthFinance@illinois.gov. In addition, a signed paper copy must be mailed to the following address at the same time as the submission of the completed Excel cost report file.
