Re: Notice of Proposed Class Action Consent Drecree and Fairness Hearing regarding the class action lawsuit N.B. v. Norwood, No. 11-C-6866

Please see the Notice of Proposed Class Action Consent Decree and Fairness Hearing and Proposed Consent Decree for the class action lawsuit N.B. v. Norwood, No. 11-C-6866, in the United States District Court for the Northern District of Illinois. You received this notification because you or your organization has been identified as a provider or other organization that serves or interacts with individuals that are likely to be members of the N.B. v. Norwood class, defined as: “All Medicaid-eligible children under the age of 21 in the State of Illinois: (1) who have been diagnosed with a mental health or behavioral disorder; and (2) for whom a licensed practitioner of the healing arts has recommended intensive home- and community-based services to correct or ameliorate their disorders.”

 

The Illinois Department of Healthcare and Family Services (HFS) is providing you or your organization with this Notice and the Proposed Consent Decree so that you or your organization will share it with anyone who may be an N.B. v. Norwood Class Member, as well as any other interested individuals.
