In conjunction with the Illinois Department of Public Health (IDPH), this Informational Notice provides an update regarding the provisional Specialized Mental Health Rehabilitation Facilities (SMHRF) licensure process.

 

Public Act 99-0712 (effective 08/05/16) provides that provisional licenses issued upon initial licensure as a SMHRF, shall expire at the end of a three year period which begins the date the provisional license is issued. At the end of the three year period, the facility must be licensed pursuant to 210 ILCS 49/4-201. 

 

Questions regarding the application process will be accepted at DPH.SMHRF@illinois.gov.  IDPH will maintain a list of questions and responses and post to the website on a bi-weekly basis as Frequently Asked Questions (FAQ’s).
