This notice informs Supportive Living Program (SLP) providers and interested parties that the Department of Healthcare and Family Services (HFS) will be accepting applications for Supportive Living Program (SLP) dementia care settings.
Dementia care settings serve persons age 65 and older who exhibit symptoms related to internal pathological changes in the brain. Those symptoms must affect intellectual and social abilities severely enough to interfere with daily functioning, in that it is unsafe for them to reside alone. Examples of diagnoses that could meet this definition include Alzheimer’s disease, dementia, Pick’s disease, brain injury or brain atrophy.
The applicant or the management company identified on the application must have healthcare experience and demonstrate experience serving the population of a dementia care setting. Applications will not be accepted in counties where currently certified Supportive Living dementia care settings are operating (Kane, Champaign, Effingham or Macon). However, currently certified Supportive Living dementia care settings may submit an application to expand their existing program. Applications will be reviewed and evaluated by HFS pursuant to the rules and regulations of Title 89 Ill. Adm. Code Part 146 Subpart B and Subpart E, including any subsequent amendments and successors thereto. General SLP rules and requirements are located in 89 Ill. Adm. Code 146 Subpart B. Specific rules and requirements for dementia care settings are located in 89 Ill. Adm. Code 146 Subpart E. Subpart E will take precedence when any program requirement is contained in both Subparts.

Chicago - Cook (including the city of Chicago), DuPage, Kane, Lake and McHenry.
South Suburb - Grundy, Kankakee, Kendall and Will.
Northwest - Boone, Carroll, DeKalb, Henry, Jo Daviess, Lee, Mercer, Ogle, Rock Island, Stephenson, Whiteside and Winnebago.
Central - Bureau, Champaign, Clark, Coles, Cumberland, DeWitt, Douglas, Edgar, Ford, Fulton, Henderson, Iroquois, Knox, LaSalle, Livingston, Macon, Marshall, McDonough, McLean, Moultrie, Peoria, Piatt, Putnam, Shelby, Stark, Tazewell, Vermilion, Warren and Woodford.
West Central - Adams, Brown, Calhoun, Cass, Christian, Greene, Hancock, Jersey, Logan, Macoupin, Mason, Menard, Montgomery, Morgan, Pike, Sangamon, Schuyler and Scott.
St. Louis - Bond, Clinton, Madison, Monroe and St. Clair.
