Chapter M-200, Handbook for Providers of Medical Equipment and Supplies – Availability on the Internet

 The department is reissuing the Handbook for Providers of Medical Equipment and Supplies, Chapter M-200, Policy and Procedures for Medical Equipment and Supplies. Providers are encouraged to review the handbook in its entirety. This handbook, in conjunction with the Handbook for Providers of Medical Services, Chapter 100, General Policy and Procedures, provides information necessary for providers to receive payment from the department for durable medical equipment and supplies.

Providers are asked to take special notice of the billing requirements related to ICD-10-CM diagnosis coding effective for dates of service on and after October 1, 2015. Appendix M-1 for claim preparation of the HFS 2210, and Appendix M-2 for claim preparation of the HFS 3797, have been updated. The HFS 2210 claim form has been revised to accommodate expanded ICD-10-CM diagnosis coding. Dates of service on or after October 1, 2015 must be billed on the revised form, which has a revision date of R-2-15 in the bottom left corner of the form. The revised form may be ordered from the Medical Forms Request page of the department’s website.
