Redeterminations (REDE) for Family Health Plan Cases with SNAP Benefits

Re: Redeterminations (REDE) for Family Health Plan Cases with SNAP Benefits

During the months of June and July, approximately 37,000 Family Health Plan cases with SNAP (formerly the Food Stamp Program) will be sent redetermination forms. These were identified as due or overdue for a review of their eligibility.

These redes are NOT part of the Illinois Medical Redetermination Process (IMRP) and are to be returned to the DHS Family and Community Resource Center (FCRC). The address of the DHS FCRC is printed on the redetermination form.

Individuals who receive the redetermination form must complete and return the form by the due date printed on the form. If the due date has already passed, the person should return the form as quickly as possible to the FCRC to avoid cancelation. If the individual’s medical case is canceled, the client can provide the form, or any other required information, within the first 3 months of cancelation, and the medical assistance case will be reinstated, if eligible. The medical redetermination also serves as a required contact for SNAP.  If the medical redetermination form is not returned, SNAP benefits may end.  If the form is later returned, SNAP eligibility will be determined from the date the form was returned. 

The person does not have to provide proof of income from Social Security or Supplemental Security Income (SSI). The FCRC can use electronic sources to get proof of the person’s income. Proof of other income should be provided with the redetermination form to avoid a delay in processing the review.

Any questions about completing the redetermination should be directed to the DHS FCRC. If the redetermination form is misplaced, the DHS FCRC can replace the form.
