Hospital Assessment for Skilled Nursing Services

Current state law requires Medicare-eligible patients to be notified of discharge at least 24 hours prior to discharge from the hospital. If the patient is pending discharge to a skilled nursing facility, the hospital must notify their Care Coordination Unit (CCU), as defined in 89 Ill. Adm. Code 240.260 , at least 24 hours prior to discharge, so that the CCU can perform an assessment (prescreening) for skilled nursing services.
Effective June 16, 2014, Public Act 98-0651 (pdf) requires that when the assessment is completed in the hospital, the CCU must provide the hospital discharge planner with a copy of the prescreening information and accompanying materials. The discharge planner is then responsible for transmitting those documents when the patient is discharged to the skilled nursing facility.
Facility providers have been instructed to contact the hospital to request the assessment if it does not accompany a person from the hospital.
