All active Region 2 LTC cases (excluding DoA and CILA) will be transferred from FCRCs to MFO.
All active Region 5 LTC cases (excluding DoA and CILA) will be transferred from FCRCs to the Macon LTC Office.
All active Region 3 LTC cases (excluding DoA and CILA) will be transferred from FCRCs to the Macon LTC Office.

This Notice informs LTC providers of the transition of active LTC cases from the local Department of Human Services (DHS) Family Community Resource Centers (FCRCs) to the centralized DHS LTC hubs located in Medical Fields Operations (MFO) and Macon LTC Office. This Notice does not apply to active Department on Aging (DoA) services-only cases or active cases for individuals residing in DHS Community Integrated Living Arrangements (CILAs) at this time.
The Department of Human Services began transitioning LTC related functions from FCRCs to the two centralized hubs in March 2014. These hubs have caseworkers that specialize in application processing and completing case actions for individuals seeking and receiving Long Term Services and Supports (LTSS).

The transition of active LTC cases for Region 4 has not been scheduled at this time.
