Nursing Home Resident's Financial and Non-Financial Information

This notice is to request your assistance in providing resident information to community-based provider agencies, various care coordination entities and managed care organizations throughout Illinois, including Aetna Better Health and IlliniCare working on the Colbert v. Quinn (07 C 4747) in Cook County and Williams v Quinn (05 C 4673) Implementation Plans.

In an effort to assist current nursing home residents who are appropriate for transition to community residency, community-based providers and care coordination entities are helping residents obtain necessary financial and non-financial documentation. Most often, a financial statement is an award letter from the Social Security Administration (SSA) but may include other official statements of resident income. Non-financial information may include original birth certificate, photo identification, and/or Social Security card.

Recently, the SSA sent all recipients an annual statement of an individual's 2014 monthly income. This statement represents and serves as an official document of income. Please ensure that any document that comes to the nursing home resident be given to the resident as his/her property and if these documents are also maintained in your business office, that they be made available to the staff of the care coordination entities upon request.

Your staff plays an important role in facilitating community-based providers and care coordination entities access to nursing home residents and their records. Access to the necessary resident information is crucial in assisting those who wish to, and are able to, transition to the community. Your continued cooperation is greatly appreciated.
