Additional Fax Line for Prior Approval Information

The purpose of this notice is to inform providers that the department has a second fax line to use when supplying additional information for a prior approval request for durable medical equipment and supplies, home health visits or occupational, physical or speech therapies.

The department has historically utilized fax number 217-524-0099 for providers to submit requests for prior approval. Providers should continue to use this number to submit initial and renewal prior approval requests.

The department now has a second fax line, 217-558-4359, that providers should use when supplying additional information to HFS staff for review of a prior approval request that was previously submitted. This second fax line is strictly dedicated to providing additional information that is necessary for further review of the original request, and may not be utilized for submitting new or renewal requests. The department will return to the provider any documentation that is sent to the incorrect fax line.  

By designating the two fax lines as outlined above, the department will streamline the review process, and reduce the time required to review the prior approval requests.
