HFS Medical Cards

Healthcare and Family Services (HFS) is pleased to announce that the mailing of the new HFS Medical Cards has been completed. With all of the changes to the HFS medical card, it is imperative providers verify medical eligibility for each date of service. The Department is requesting that providers use one of the following resources to verify an individual’s eligibility:

Please do not refuse to provide services due to an individual’s inability to produce a medical card. Also, HFS is reminding providers that they may not charge participants for eligibility verification.

Providers choosing to check eligibility via telephone must use one of the numbers listed above and should not contact the billing staff to check eligibility.

Please do not request that the patient go to the Family Community Resource Center (FCRC) for assistance. The FCRC no longer issues temporary medical cards or help with medical card issues. For help with medical card issues or to get a lost medical card replaced, customers may call:

  • HFS at 1-800-226-0768 (TTY: 1-877-204-1012).
