Managed Care Health Plan Choices

The purpose of this notice is to inform providers of the health plan choices available to Healthcare and Family Services (HFS) and All Kids participants by county.

Most HFS or All Kids medical cardholders are required to select a Primary Care Provider (PCP) and Health Plan for their medical home. Statewide, most participants must enroll in the Primary Care Case Management program, called Illinois Health Connect, for their medical home. Clients living in counties where voluntary managed care is available may choose Illinois Health Connect or a Managed Care Organization for their medical home. Seniors and adults with disabilities living in the Integrated Care Program (ICP) pilot counties are required to choose a PCP in one of the Integrated Care Program Managed Care Organizations.
