Re: Addendum to June 27, 2011 Provider Bulletin Reporting of Other Operating Physician Name and NPI

 

For all institutional claim format transactions, the Operating Physician Name and NPI are required when a surgical procedure is listed on the claim.

The Other Operating Physician Name and NPI denote the individual assisting the Operating Physician. This Other Physician can only be reported when the Operating Physician information is also sent on the claim. The reporting of the Other Operating Physician without the Operating Physician will result in a non-compliant transaction.
