Managed Care Health Plan Choices

The purpose of this notice is to inform providers of the health plan choices available to Healthcare and Family Services (HFS) and All Kids participants by county.  

Most HFS or All Kids medical cardholders are required to select a Primary Care Provider and Health Plan for their medical home. Statewide, most participants must enroll in the Primary Care Case Management (PCCM) program, called Illinois Health Connect, for their medical home. Clients in the voluntary managed care counties may choose the PCCM program or enroll with a Managed Care Organization (MCO) for their medical home. Older adults and adults with disabilities living in the Integrated Care Program (ICP) pilot area must choose a PCP in one of the Integrated Care Program HMOs. The following chart shows the managed care health plan choices available by county.
