Transition of Medicaid clients previously eligible for disease management program (Your Healthcare Plus) to new Integrated Care Program & Illinois Health Connect Program​

 

The Illinois Department of Healthcare and Family Services (HFS) is in the process of launching a new Integrated Care Program for approximately 40,000 older adults and adults with disabilities in the counties of DuPage, Kane, Kankakee, Lake, Will and Suburban Cook who are eligible for Medicaid but not eligible for Medicare. Please refer to Informational Notice on the HFS Web site dated February 10, 2011, for more detail.

Effective May 1, 2011, this population is subject to mandatory enrollment in the Integrated Care Program. Approximately 28,000 of these 40,000 enrollees have been involved in a disease management program called Your Healthcare PlusTM. After enrollment in the Integrated Care Program, members will no longer be eligible for Your Healthcare Plus programming, and will be eligible to receive their disease management services from the Integrated Care Program. HFS along with the Your Healthcare Plus program and the Integrated Care Program are coordinating a smooth and seamless transition for these members.

 

The remainder of the approximately 250,000 members not enrolling in the Integrated Care Program, and previously eligible for disease management services, will continue eligibility with the Your Healthcare Plus program through June 30, 2011. Beginning July 1, 2011, the Your Healthcare Plus program will end and formerly eligible disease management members will continue to receive primary health care services and support through their medical home and with the Illinois Health Connect program.
