Dental Clinic Grant Application

The Illinois Department of Healthcare and Family Services (HFS) is pleased to announce a grant funding opportunity for development of comprehensive dental clinics for dental care services.

 

Eligible applicants include:

  • Local Health Departments (LHDs)

  • Federally Qualified Health Clinics (FQHCs)

  • Rural Health Clinics (RHCs)

The primary purpose of these new or expanded dental clinics will be to increase dental access for low-income and HFS beneficiaries. The dental clinic must be willing to accept out-of-area clients who need dental services, including emergency services for adults and children referred under Early and Periodic Screening, Diagnosis and Treatment (EPSDT). Medically Underserved Areas (MUAs) and Health Professional Shortage Areas (HPSAs) shall receive priority for grants under this program.

Qualified applicants may apply for grant funding up to $100,000 in total to be distributed in two payments over a two-year grant period.

Pursuant to an appropriation made by the 96th General Assembly as set forth in Public Act 96-0039, Article 13, Section 5, the sum of $2,000,000 is available to fund the Dental Clinic Grant Program established by 305 ILCS 5/12-4.39. Additionally, HFS has appropriated additional funds for dental clinic grant development and expansion. The total amount of available funds through this grant opportunity is $3,000,000, of which $180,000 is already committed.

  • $2,000,000 for funding capital projects

  • $820,000 for funding operational projects

  • $180,000 for funding the second year of six existing dental grants awarded in FY10

Dental Grants will be evaluated based on the following criteria:

  • Clear justification of the need for Medicaid access

  • Located in a Medically Underserved Area (MUA) or Health Professional Shortage Areas (HPSA)

  • Measurable and realistic program goals and objectives

  • Clear statement of how the proposal will make "Dental Home" a reality for HFS children beneficiaries in the Grant Applicant's jurisdiction

  • Willingness to accept out-of-area HFS beneficiaries

  • Adequate capacity to provide a full range of services, including treatment (include the scope of treatment capacity)

  • Ability to accept referrals from community partners (e.g., Medical Home) and make referrals to specialists as needed

  • Ability to provide outreach to unserved HFS beneficiaries
