Illinois MFP Demonstration Program – Outreach by Ombudsmen

Ombudsmen across Illinois have been designated to assist with the identification of persons currently residing in nursing homes who are interested in possible transition to the community under the Money Follows the Person (MFP) Demonstration. The Department of Healthcare and Family Services (HFS) is the lead agency in the Illinois MFP Demonstration, which operates under the auspices of the federal Centers for Medicare and Medicaid Services. The MFP Program is a major federal initiative to assist states in rebalancing long term care systems. HFS has partnered with the Department on Aging (DoA) and the Department of Human Services (DHS) and its divisions of Mental Health (DMH) and Rehabilitation Services (DRS).
Transition coordinators from community agencies under contract with DoA, DMH or DRS may have already visited your facility to contact and assess residents regarding MFP. The Ombudsmen have been asked to provide additional and extended outreach by speaking to resident councils, residents and their families and friends about MFP. Self-referrals by residents or family members for MFP are welcome.
HFS and its state partners ask that all nursing home staff facilitate the efforts of ombudsmen in reaching nursing facility residents regarding MFP. To support this important Medicaid initiative, please facilitate access to residents, staff caregivers, and records as required for both Ombudsmen and the Transition Coordinators working with potential MFP candidates.
Referrals of potential MFP transition candidates by nursing facility staff are most welcome! We know
that facility staff knowledge and concern for residents are a significant resource. Nursing facilities are important partners in this process and will continue to play a critical role in the long term care service system for the future.
