Re: SLF Application Extension
 

On August 12, 2008, the Department of Healthcare and Family Services (HFS) announced that HFS will be accepting applications for the Supportive Living Program (SLP) in targeted geographic areas until January 15, 2009.  This notice is announcing that the deadline for accepting applications is being extended from January 15, 2009 to March 2, 2009. 

As a reminder, submissions for senior developments will be accepted for projects serving individuals in the geographic areas listed below.  Applications for SLFs serving persons with physical disabilities age 22-64 may be submitted to serve individuals in any location.
