Re: Medical Home Health Plans Choices

 

The purpose of this notice is to inform providers of the health plan choices most HFS or All Kids medical cardholders have by county.   

Most HFS or All Kids medical cardholders are required to select a Primary Care Provider and Health Plan for their medical home.  Statewide, most HFS or All Kids cardholders must enroll in the Primary Care Case Management (PCCM) program for their medical home.  Clients in the voluntary managed care counties may opt out of the PCCM program and enroll with a Managed Care Organization (MCO) for their medical home.  The following chart shows medical home health plan choices by county.
