Revision to the Billing Instructions for the Medicare Crossover Invoice (HFS 3797)

​Re: ​Revision to the Billing Instructions for the Medicare Crossover Invoice (HFS 3797)

The purpose of this notice is to inform providers of changes to the billing instructions for the HFS 3797, Medicare Crossover Invoice.  The changes made were regarding the reporting of the National Drug Code (NDC).  Previous instructions did not require the reporting of a procedure code (Box 10E) when an NDC (Box 11) was being reported.  Providers now must report a procedure code in addition to an NDC.  The affected items are designated by “=” signs to the left.  Revised billing instructions are posted with this provider notice at:
