Prior Approval Changes for Polycarbonate Lenses

The department is pleased to announce that providers will no longer be required to request prior approval for polycarbonate lenses for children through age 20.
In an informational notice dated July 14, 2006, the department identified the process for requesting prior approval and billing for polycarbonate lenses. Effective with service dates on and after August 17, 2007, the department eliminated the prior approval requirement on polycarbonate lenses for children through age 20.
The Illinois Department of Corrections eyeglasses laboratory will automatically fabricate all eyeglasses with polycarbonate lenses for children through age 20. Providers should complete paper claims according to billing instructions contained in the Handbook for Providers of Optometric Services, Appendix 1, on the department’s Web site at <http://www.hfs.illinois.gov/handbooks/chapter200.html>.
Prior approval is still required for patients age 21 and older. Please continue to follow the prior approval and billing requirements in the July 14, 2006, notice for this patient population.
