Supportive Living Facility Provider Handbook Update - Form HFS 3869, Medication Error Report​

 

 The attached SLF Handbook update informs facilities of a new form.  Form HFS 3869, Medication Error Report, has been added to the handbook to provide supportive living facilities with a uniform format for reporting errors in medication administration.  Instructions for the completion of this form are also included. 

 

 

All medication errors must be reported using this new format effective the date of this release.  The completed form must be included in the resident’s files for Department review.

Medication errors that result in a hospitalization due to an adverse reaction must be reported to the facility’s HFS Regional Supervisor within 24 hours of the discovery of the medication error.

HANDBOOK REVISIONS

Remove pages 23 and Appendix C Index with the attached pages.

Place Appendix C-20 in the reserved place between C-19 and C-21.
