Re:​ Disease Management Program – Your Healthcare Plus™​

Effective July 1, 2006, the department implemented a new disease management program named Your Healthcare Plus™.  Your Healthcare Plus  will support HFS providers, including nursing facilities, with the medical management of Medicaid-eligible residents with complex chronic illnesses.  Please note that this program serves only Medicaid-eligible residents of your facility. McKesson Health Solutions will administer this program.

Over the next several weeks, McKesson Health Solutions will be contacting residents in your facility who are eligible for the program and their physicians.  Provider and resident participation are voluntary. The Your Healthcare Plus program is:

  • Available at no cost to adults with disabilities who suffer from chronic or complex medical conditions, to children and adults with persistent asthma and to high frequency emergency room users.

  • Designed to benefit medical providers through the added support of Your Healthcare Plus registered nurses, pharmacists, behavioral health specialists and social workers.

  • Intended to assist LTC staff by complementing the care they provide to their residents.

Since Your Healthcare Plus is a voluntary program, you are a valuable partner in helping to create a successful program by encouraging your staff to support the participation of your eligible residents.
