Re: Supportive Living Facility Provider Handbook Update
– Revised Form 3732, Involuntary Discharge Notice of Appeal and Request For Hearing
– Revised Rate Map

 

The attached Form HFS 3732, Involuntary Discharge Notice of Appeal and Request For Hearing, has been revised.

 

The methods by which an appeal may be submitted have been expanded. The methods for submitting the appeal are described in the first paragraph of Form 3732.

 

The signature of the appellant and date appeal is made is now required on the revised form. The address the appeal is to be sent to has been moved from Page Two to the first paragraph on Page One.

 

The attached rate map has been updated with the SLF rates effective October 1, 2005.
