Re: Long Term Care Provider Handbook (SLF) Update - Revised Form HFS 1156, LTC Facility Notification

 

Effective June 1, 2005, facilities are no longer required to complete Form HFS 2448, Physician Certification, when admitting a resident.

The revised Form HFS 1156, Long Term Care Facility Notification, now contains the necessary information for an admission to be processed by the Department of Human Services local office caseworker without a Form HFS 2448. The required information formerly contained in Form HFS 2448, can now be found in Section 1 of Form HFS 1156.

 

The admitting diagnosis continues to be required and must be identified by the appropriate ICD - 9 code and not a description. This diagnosis code will still be provided by the resident's physician. Although the physician's name and identification number continues to be required, the physician's signature is not.

The name of the Department of Public Aid (DPA) has changed to Healthcare and Family Services, HFS for short.

 

You will soon see the new name on forms, letters and the Internet. Until existing form supplies are exhausted, however, we will continue to use materials bearing the former DPA name and logo. Either version of documents should be considered official regardless of the department name they bear.

 

Only our name has changed. HFS's programs, staff, office locations, mailing addresses and phone numbers remain the same as DPA's.

 

We believe our new name more clearly reflects our mission of service to the State of Illinois including providing access to health care for over 2 million Illinoisans. In our work, we deeply appreciate your participation in our programs. We hope this change poses no inconvenience or confusion for you.
