Handbook for Providers of Transportation Services January 2005 Update

The purpose of this bulletin is to inform providers of the following updates to the Handbook for Providers of Transportation Services.

• With the passage of Senate Bill 2225, enacted in 2002, language has been added to the handbook regarding the definition of non-emergency transportation providers, fingerprinting/background checks and a 180 day probationary enrollment.

• A new Appendix T-4, containing a sample Uniform Trip Ticket that may be used to document pertinent trip information, has been added. This sample trip ticket is not required or issued by the department. However, it does contain information that can assist providers in fulfilling their record requirements.

• The department’s transportation approval agent’s name has changed from DynTek Services Inc. to First Transit Inc.

• Several updates to the Provider Participation and Transportation Reimbursement sections, including elimination of the prepayment review requirement for emergency helicopter claims.
