Effective October 16, 2004, the department will close procedure code L3218 for reimbursement to podiatrists. This code, which became obsolete in 2002, will be replaced with procedure code L3260 (surgical boot/shoe, each) for dates of service October 16, 2004, and after. L3260 will require prior approval and will be reimbursed up to the state maximum rate of $64.39.
