Home and Community-Based Service Alternatives for Patients Needing Long Term Care

​Re: ​Home and Community-Based Service Alternatives for Patients Needing Long Term Care

 

Persons who require long term care in a hospital, nursing facility or Intermediate Care Facility for the Developmentally Disabled may be eligible for home and community-based service (HCBS) alternatives that will enable them to avoid institutionalization. Services that may be available vary, but include homemaker and adult day care.

 

 

Illinois has HCBS waiver programs serving seven distinct populations of persons with chronic care needs. To find out more about these programs, please access the Department of Public Aid's website at <http://www.dpaillinois.com/hcbswaivers/> or call the number listed below. The HCBS waiver programs include:

 

Children who are Medically Fragile and Technology Dependent  

  1. Children who are Medically Fragile and Technology Dependent
  2. Persons with Disabilities
  3. Persons with Brain Injury
  4. Persons with HIV/AIDS
  5. Adults with Developmental Disabilities
  6. The Elderly
  7. Persons who may live in Supportive Living Facilities

The Department encourages physicians and discharge plnners to consider these services as they work to assist patients and their families to plan for long term care.
