Publicly Owned Transportation Supplemental Payments for Providers – Ground Emergency Medical Transportation (GEMT)

This notice is to inform providers enrolled in the Ground Emergency Medical Transportation (GEMT) program that payment for Fiscal Year 2026, Quarter 2 (October 1, 2025 – December 31, 2025) is due by April 30, 2026.

Preferred Payment Method: ACH Direct Debit

A provider-initiated ACH Direct Debit is the preferred method of payment. Providers may request Form 3881 by contacting HFS at: 
HFS.GEMT@Illinois.gov 

Alternative Payment Method: Check

If you choose not to use the ACH process, you may remit payment via check. 
Please make the check payable to the Illinois Department of Healthcare and Family Services and include the invoice number and note “GEMT Payment” on the check. Mail to:
