Provider Notice issued 12/23/2025
- Each resident must review, sign, and date their contract/lease every year.
- If no changes have been made to the contract/lease from the previous year, the resident may sign and date directly beneath their prior year’s signature.
- If any changes have been made—regardless of how minor—they must be clearly noted, and the resident must provide a new signature and date acknowledging the updated terms.

This notice is to inform all Supportive Living Program (SLP) providers that resident contracts/leases must be re-signed and dated on an annual basis.
Annual Signature Requirement
Please refer to 89 Ill. Adm. Code Section 146.240 (a) and (d) for additional information regarding resident contracts.
This requirement ensures continued clarity, transparency, and compliance with HFS, program, and SLP building expectations.
