This notice serves as a reminder that Supportive Living Program (SLP) providers are required to complete a conversion screening when individuals transition from private pay to Medicaid.
An individual who enters the SLP as private pay and later converts to Medicaid while in the SLP must then have the preadmission screening process completed. The SLP should request a screening from the Care Coordination Unit (CCU).
89 Ill. Adm. Code 146.220(c) requires the conversion screening/determination of need (DON) screening to be completed prior to the conversion to Medicaid. If there is a late conversion screening, the admission date will be updated to reflect the screening date. The admission/payment start date cannot be before the conversion screening date.
