Publicly Owned Transportation Providers – Ground Emergency Medical Transportation (GEMT) Supplemental Payments

This notice is to inform providers enrolled in the Ground Emergency Medical Transportation (GEMT) program that payment for Fiscal Year 2026, Quarter 1 (July 1, 2025 – September 30, 2025) is due by January 30, 2026.
Preferred Payment Method: ACH Direct Debit
A provider-initiated ACH Direct Debit is the preferred method of payment. Providers may request Form 3881 by contacting HFS at:
HFS.GEMT@Illinois.gov
Alternative Payment Method: Check
If you choose not to use the ACH process, you may remit payment via check.
Please make the check payable to the Illinois Department of Healthcare and Family Services and include the invoice number and note “GEMT Payment” on the check.
