Tailored Care Management for Williams & Colbert Class Members Listening Session Additional Opportunity to provide Feedback

The Illinois Department of Healthcare and Family Service (HFS) seeks to gather feedback on how current transition programs and services are working for class members and Medicaid customers and where they see opportunities for improvement. The feedback will inform HFS' approach to implementing an integrated, person-centered transition program, with a single point of accountability, to improve service coordination, service delivery, and outcomes for high-need populations.
